Documentation for healthcare professionals

If you are a healthcare professional and one of your patients is interested in donating neurological tissue to the brain donation program, he/she must complete the following documentation:

• If the donation of neuronal tissue is to be made by a living donor or their legal representative, the documentation is the followin:
  • Consent for donating brain tissue to the Biobank whilst living.
  • Autopsy Request
• The documentation to be completed by the family to donate a neuronal tissue sample from a dead donor is the following:
  • Informed consent for post-mortem donation
  • Autopsy Request
